Patrick Horn served on the crew of the SF to SB Relay, an attempt to swim 339 miles (545.5 km) from San Francisco to Santa Barbara along the California coast. Horn helped Captain Vito Bialla and swimmers Phil Cutti, Dave Holscher, Joe Locke, Kim Chambers and Zack Jirkovsky swim non-stop for 6 days before the swim was unexpected halted due to safety when the athletes encountered bloom after bloom of venomous jellyfish.

Horn was also a crew member of the Nighttrain228 together with the Night Train Swimmers who did a California Coastal Swim, a 228-mile relay swim under the direction of captain Vito Bialla with Hal McCormick and Patrick Horn as well as relay members Phil Cutti, David Holscher, Zach Jirkovsky, Luane Rowe, Blair Cannon, and Grace van der Byl with Walter Bean Scott as the alternate swimmer between 22-26 August 2013.
